保泰公司醫療保險計劃限時優惠條款及細則
- 保泰公司醫療保險計劃限時優惠 (「此優惠」) 只適用於由 2023 年 10 月 1 日起至 12 月 31 日期間(包括首尾 2 天)(「優惠期」)。
- 此優惠只適用於成功投保保泰公司醫療保險 Basic Silver 或 Basic Gold 計劃之保單(「指定公司醫療保險計劃保單」)。
- 在此條款及細則約束下,投保人之合資格保單須符合以下項目 3.1 - 3.4 之條件方可獲 MyDoc 遙距視像會診服務。
3.1 須於優惠期內遞交指定公司醫療保險計劃保單申請;
3.2 投保公司須從未持有任何保泰公司醫療保險計劃或保泰團體醫療保險之保單;
3.3 合資格保單必須有至少 5 名員工或以上。
3.4 申請須獲 Bowtie 成功批核;合資格保單必須維持生效期最少一個⽉。 - 此優惠不可更換、退回、轉換其他禮品、轉售,或與其他推廣優惠及/或現金券同時使用。
- 客戶可於有效期內根據 Bowtie 客戶平台的 MyDoc 指引無限次使用視像會診服務(免診金及藥物費用)。服務包含 2 日基本藥物、藥物運費及醫生紙(如適用)。
- 遙距視像會診受 MyDoc 的條款與細則約束。如有任何有關遙距視像會診服務之爭議,MyDoc 保留最終決定權。
- 此服務由 MyDoc 提供。由此服務提供的服務所產生之任何責任,保泰人壽保險有限公司概不承擔。如對此服務有任何問題,請向 support@my-doc.com 查詢。
- 「視像會診」服務只適合症狀輕微及非緊急病患。使用視像會議軟件提供醫療服務有機會不能等同於病人與醫生直接接觸與問診。如病情嚴重,請聯絡你的家庭醫生或到附近的醫院進行面診。
- Bowtie 將會在符合資格的保單維持有效期一個月後的 30 天內,發放 MyDoc 視像會診服務到受保人帳戶,並向所登記的電子郵件地址發送通知電郵。受保人可登入 app.bowtie.com.hk,選擇健康服務「視像會診」使用服務。
- 任何因電腦、網路等技術問題導致投保人所遞交的資料有所遲延、遺失、錯誤的情況,Bowtie 毋須負上任何責任。所有與投保有關之日期及時間 (包括但不限優惠期) 均以 Bowtie 的時間紀錄為準。
- Bowtie 保留在不作任何事前通知的情況下修改、暫停或取消此優惠及修改其條款及細則的權利。如對以上優惠有任何爭議, Bowtie 保留最終決定權。
Bowtie Corp Medical Insurance Plan Limited Time Offer Terms & Conditions
- Bowtie Corp Medical Insurance Plan Limited Time Offer (“the Promotion”) is valid from 1 October 2023 till 31 December 2023, both days inclusive (“the Promotion Period”).
- The Promotion is applicable upon successful enrolment of the Bowtie Corp Medical Insurance Basic Silver or Basic Gold Plan (“the Designated Plan)”).
- Subject to these terms and conditions, an Eligible Policy must fulfill the following conditions (3.1 – 3.3) in order to receive MyDoc Telemedicine Service.
3.1 The application for the Designated Plan is successfully enrolled during the Promotion Period;
3.2 The policyholder has never held any Bowtie Corp Medical Insurance Plan or Bowtie Group Insurance Plan;
3.3 An Eligible Policy should have at least 5 staff members or above.
3.4 To be eligible, the application must be approved by Bowtie, and the qualified policy must remain in effect for a minimum of one month. - The Promotion cannot be changed, returned, exchanged for other gifts, sold or transferred to a third party, or used in conjunction with other promotional offers.
- The Service entitles customers to use the Telemedicine Service in accordance with the instructions of MyDoc shown in the Bowtie customer portal within the relevant Service contract period for unlimited number of times (no charge for consultation and medicine). The Service also includes 2-day basic medication, delivery, and sick leave certificate (if applicable).
- The Telemedicine Service is subject to the Terms and Conditions of MyDoc. In case of any disputes regarding the service, the decision of MyDoc shall be final and binding.
- The Telemedicine Service is provided by MyDoc. Bowtie will not be liable for any liability arising from these services or other services provided by MyDoc and its underlying service quality. Please email support@my-doc.com for any queries about the Telemedicine Service.
- Telemedicine Service is only for mild and non-urgent medical conditions for patients in Hong Kong. The use of video technology to deliver healthcare and educational services may not be equivalent to direct patient-to-physician contact. In case of emergency, please consult your family doctor or visit the nearest hospital.
- The Service will be automatically added to the user’s account within 30 days after the qualified policy has been in effect for the first month. The insured persons can log in to app.bowtie.com.hk and select “Telemedicine Service” under Health Service to use The Service.
- Bowtie will not be liable for any matter or loss in relation to submission delay, loss, and any information transmission error due to technical problems including but not limited to any computer or internet network issues. All dates and times related to the Promotion (including but not limited to the Promotion Period) are recorded according to the reception of Bowtie’s server and computer records.
- Bowtie reserves the right to modify, suspend, or terminate this set of Terms and Conditions at any time without prior notice. In case of any dispute, Bowtie reserves the right to the final decision.
