第一階段：於活動期間填妥表格並完成提交其他保險公司的有效團體醫療保險計劃保單資料，可獲 HK$200 HKTVmall 電子購物禮券。

1. 活動截止日期為 2025 年 12 月 31 日。

2. 此活動優惠只適用於 Bowtie 以外其他保險公司的團體醫療保險現有客戶，參加者須於開放報名時間內提交表格，逾期無效。

3. 在此條款及細則約束下，合資格登記公司須符合以下項目 3.1 - 3.5 之條件方可免費享有 HK$200 HKTVmall 電子購物禮券：

3.1. 須為現正從事人力資源（HR）或相關工作的人士；
3.2. 須持有其他保險公司仍然生效之團體醫療保險計劃保單；
3.3. 於優惠期內成功提交登記表格；
3.4. 根據確認電郵指示提交有效保單證明文件。有效保單證明文件包括保單文件及僱主帳戶截圖，須清楚顯示保單持有人、保單號碼及保單有效期；
3.5. 須於與 Bowtie 公司醫保專員聯絡後 14 天内提交所屬公司的個人名片或相關公司資料以核實身份。

4. 名額有限，每間公司或機構最多 1 個名額，只限參與此活動及獲得禮券 1 次。參與者須以公司電郵登記。主辦機構保留隨時查核參加者資格的權利。

5. 請確保填寫電郵地址準確無誤，以便接收活動的最新資訊及禮遇。如資料有誤，相關電郵將不會重發。

第二階段：其他保險公司的現有團體醫保客戶成功投保 Bowtie 公司醫保，可獲 HK$600 HKTVmall 電子購物禮券。

1. 活動截止日期為 2025 年 12 月 31 日。

2. 此優惠只適用於成功投保保泰團體及公司醫療保險計劃之保單（「指定公司醫療保險計劃保單」）。

3. 在此條款及細則約束下，投保人之合資格保單須符合以下項目 3.1 - 3.4 之條件方可獲 HK$600 HKTVmall 電子購物禮券（禮券）。

3.1 須於優惠期內遞交指定公司醫療保險計劃保單申請；
3.2 指定公司醫療保險計劃包括 Basic Silver、Basic Gold、Pro Bronze、Pro Silver、Pro Gold、Pro Platinum 以及自選專屬公司醫保計劃。
3.3 投保公司須從未持有任何保泰團體及公司醫療保險計劃或保泰團體醫療保險之保單；
3.4 申請須獲 Bowtie 成功批核；合資格保單必須維持生效期最少 3 個⽉。

4. 名額有限，每間公司或機構最多 1 個名額，每階段只限參與此優惠及獲得禮券 1 次。參與者須以公司電郵登記。

5. 參與者須持有其他保險公司仍然生效之團體醫療保險計劃保單。第二階段參與者毋須同時參與第一階段，只須於投保時向 Bowtie 出示有效證明，主辦機構保留隨時查核參加者資格的權利。

6. Bowtie 將會在符合資格的保單維持有效期 3 個月後的 75 天內，向登記的電子郵件地址發送換領電郵。保單持有人可以直接使用兌換電郵提供的聯繫信息進行兌換。

Bowtie Corporate Medical Insurance Limited-Time Switching Offer Terms and Conditions

Phase 1: Register and complete verification with your existing group medical insurance policy information during the promotion period to receive a HK$200 HKTVmall e-voucher.

1. The offer is valid until 31 December 2025.

2. This offer is only applicable to existing group medical insurance customers of insurance companies other than Bowtie. Participants must submit the form within the promotion period; late submissions will not be considered.

3. To qualify for the HK$200 HKTVmall e-voucher ("voucher") under these terms and conditions, the participant must fulfil the following conditions (Section 3.1 - 3.5):

3.1. Must be currently engaged in Human Resources (HR) or related work;
3.2. Must hold a valid group medical insurance plan policy from another insurance company;
3.3. Successfully submit the registration form during the promotion period;
3.4. Submit valid verification documents as per instructed by our verification email. Valid verification documents include policy documents and employer account screenshots, clearly showing the policyholder, policy number, and policy effective period;
3.5. Must submit personal business card or relevant company information within 14 days after contacting Bowtie's company medical insurance specialist for identity verification.

4. Each company or organisation is limited to one quota and can participate in this activity and receive the voucher only once. Participants must register with a company email.

5. The organiser reserves the right to verify participants' eligibility at any time.

6. Please ensure the provided email address is valid to receive the latest information and communications regarding the offer. If the information is incorrect, related emails will not be resent.

7. Bowtie will send a redemption email to the registered email address in January 2026 after confirming the existing policy information, personal business card, or relevant company information.

Phase 2: Switch to Bowtie Corp Medical Insurance within the promotion period to receive a HK$600 HKTVmall e-voucher.

1. The offer is valid until 31 December 2025.

2. This offer is only applicable to successfully enrolled Bowtie Corp Medical Insurance policies ("designated Corp Medical Insurance Plans").

3. To qualify for the HK$600 HKTVmall e-voucher (“voucher”) under these terms and conditions, the participant must fulfil the following conditions (Sections 3.1 - 3.4):

3.1 Must submit the designated Corp Medical Insurance Plan application during the promotion period;
3.2 Designated Corp Medical Insurance Plans include Basic Silver, Basic Gold, Pro Bronze, Pro Silver, Pro Gold, Pro Platinum, and Tailor-made corporate medical insurance plans;
3.3 The applying company must have never held any Bowtie Corp Medical Insurance policy;
3.4 The application must be successfully approved by Bowtie and the policy must remain effective for at least 3 months.

4. Each company or organisation is limited to one quota and can participate in this offer and receive the voucher only once per phase. Participants must register with a company email.

5. Participants must hold a valid group medical insurance plan policy from another insurance company. Phase Two participants do not need to participate in Phase One simultaneously but must present valid proof to Bowtie when enrolling. Bowtie reserves the right to verify participants' eligibility at any time.

6. Bowtie will send a redemption email to the registered email address within 75 days after the eligible policy has remained effective for 3 months. Policyholders can redeem directly using the information provided in the redemption email.

7. Gift quantities are limited and available on a first-come, first-served basis. Bowtie reserves the right to substitute with other gifts or offers of equal or similar value.

8. This offer cannot be exchanged, returned, converted to other gifts, resold, or used in conjunction with other promotional offers and/or cash vouchers.
