Eye Examination Offer Terms and Conditions

1. The Optical 88 Comprehensive Eye Examination is provided by Optical 88 Ltd (“Optical 88”). Terms and conditions apply: 
   
   1. Bowtie will only offer one eye examination for the insured person
   2. The Eye Examination is only available to Eligible Policy insureds aged 4 or above and is non-transferable.
   3. The Eye Examination includes
      
      1. Ocular and general health history
      2. Refraction test
      3. Intra-ocular pressure measurement (By non-contact tonometry)
      4. Binocular vision Assessments (E.g. stereopsis and accommodation test)
      5. Colour vision test (By Ishihara test)
      6. External ocular health and fundus examination
      7. Consultation and recommendation
      8. Detailed ophthalmology report (only applicable to comprehensive ophthalmology examination)
      9. Summary of optometrist’s clinical findings and observations from exam, includes retinal photo when requested by patient, approximate 1-week processing time. (report provided upon request)
   4. This may involve pupil dilation during the comprehensive eye exam, you may experience blurry vision after the comprehensive eye exam. You should follow their instructions/advices and terms & conditions which may apply from their professionals.
   5. If the policyholder cancels or surrenders the Eligible Policy before the end of the first policy year, Bowtie reserves the right to recover the fee of the Eye Examination. 
   6. Bowtie will send out a redemption email after 45 days after the policy becomes effective. Customers can enjoy the the Eye Examination according to the instructions in the redemption email
   7. The Offer is subject to Optical 88 availability on a first-come, first-served basis.
   8. The redemption email will not be re-issued if lost and will be invalid if found defaced or altered. 
   9. The Eye Examination is available to the insured person only. It cannot be transferred or resold to others and cannot be exchanged, refunded, converted to other gifts, or used in conjunction with other offers. 
2. Bowtie is not the supplier of the Eye Examination and makes no representation or guarantee as to the quality of the Eye Examination and is not responsible for any matter relating to the Offer. Optical 88 is the supplier of the Eye Examination and is responsible for all issues on the provision, obligations and disputes relating to the Eye Examination. Please contact the supplier or third party directly for any queries.
3. Bowtie and Optical 88 reserve the right to substitute the Eye Examination (or part of the Eye Examination) with an alternative gift with equal monetary value and/or specification.
4. Bowtie will not be liable for any matter or loss in relation to submission delay, loss, and any information transmission error due to technical problems including but not limited to any computer or internet network issues. All dates and times related to the Offer (including but not limited to the Promotion Period) are recorded according to the reception of Bowtie’s server and computer records.
5. Bowtie reserves the rights of the final decision, including suspending, terminating or changing details of the Offer and its terms and conditions without prior notice.
