Bowtie 12-month Comprehensive Dental Services Terms and Conditions

1. 12-month Comprehensive Dental Services (“the Dental Services”) are provided by the third party provider and sponsored by Bowtie. Terms and conditions apply:
   a. New customer who successfully applies with the designated promo code for designated products will automatically become a BowtieGo member and can enjoy the Dental Services.
   b. The Dental Services is only available to Eligible Policy insureds or policyholders aged 6 or above and is non-transferable.
   c. The Dental Services includes：
   - Basic dental services for twice (Oral examination and consultation, Scaling and polishing and Intraoral x-ray)
   - Dental Procedure Benefits: 20% savings on the retail price, services including:
   (A) Restorative dentistry (Composite fillings; diastema; crowns and bridges; and denture);
   (B) Endodontics (Root canal treatment; dressings; and temporary fillings);
   (C) Periodontology (Root planning); and
   (D) Other dental services (Non-surgical extraction; non-surgical wisdom tooth extraction; mouth guard; fluoride treatment; and non-surgical drainage of abscess).
   - Cosmetic Dentistry Benefits:HK$2,000 savings on minimum spending of HK$20,000 for cosmetic dentistry treatment including:
   (A) Clear aligner treatment;
   (B) Veneers; or
   (C) Bleaching.
   d. The Dental Services is valid for a period of 12 months from the date of issuance. Customers need to redeem the Dental Services within the effective period. Otherwise it will be invalid and will not be reissued.
   e. The Dental Services is subject to the actual operating conditions of the third party provider. Customers are required to follow the instructions provided in the redemption email issued by Bowtie to make appointments and redeem the Dental Services. 
2. The Dental Services cannot be exchanged, returned, exchanged for other gifts, or used in conjunction with other promotional offers.
3. Bowtie is not the provider of the Dental Services, which are provided by the third party provider. Bowtie is not responsible for any liability arising out of the Dental Services or other services provided by the third party provider, nor for any problems or complaints about the service or its related service quality, please contact or inquire directly with the relevant provider or third party.
4. If the policyholder cancels or surrenders the Eligible Policy before the end of the first policy year, Bowtie shall have the right to recover the fee of the Dental Services. 
5. Bowtie and the third party provider reserve the right of final decision on the above promotions and reserve the right to suspend, terminate, or substitute the Dental Services with an alternative gift with equal monetary value and/or specification and change the details of the Offer without prior notice.
6. Bowtie will not be liable for any matter or loss in relation to submission delay, loss, and any information transmission error due to technical problems including but not limited to any computer or internet network issues. All dates and times related to the Offer.
