Bowtie 全面眼科視光檢查之條款及細則約束

  1. 眼鏡 88 全面眼科視光檢查服務由眼鏡 88 有限公司 (「眼鏡 88」) 所提供,此服務由 Bowtie 贊助。客戶可根據指示享用優惠。受以下條款約束:
    a. 此服務只限年滿 4 歲或以上的合資格保單受保人享用,不可轉讓。
    b. 此服務項目包括:
        i.     眼睛及身體健康病歷、
        ii.    屈光檢查、
        iii.   量度眼內壓(使用非接觸式眼壓計)、
        iv.   雙眼視覺功能檢查(如:立體視覺及調節力測試)、
        v.    色覺普查(使用石源測試)、
        vi.   眼睛前部及視網膜檢查(包括淚水、角膜、晶狀體、黃斑區及視覺神經健康)、
        vii.  提供諮詢及專業建議、
        viii. 眼科詳細報告、
        ix.   由註冊視光師的臨床檢驗及觀察結果,包括視網膜照片 (需時七個工作天,客戶需當場提出報告需求)
    c. 此服務須視乎眼鏡 88 實際營運情況而定,客戶需依照由 Bowtie 發出之換領電郵內的指引,預約指定眼鏡 88 專業護眼中心進行檢查。
    d. 全面眼科視光檢查過程中或會使用放瞳藥水。用藥期間會令瞳孔放大,令視力短暫下降及畏光。你應根據專業眼科視光師的臨床建議和遵從有關條款及細則。
  2. 此服務不可更換、退回、轉換其他禮品,或與其他推廣優惠同時使⽤。
  3. Bowtie 並非此服務之供應商,此服務由眼鏡 88 提供。由此服務或其他眼鏡 88 提供的服務所產生之任何責任,及有關服務之任何問題或投訴或其相關服務質素,Bowtie 概不負責,請直接與有關供應商或第三方聯絡或查詢。
  4. 如保單持有人於合資格保單的首個保單年度終結前取消保單或退保,Bowtie 有權追討有關此眼鏡 88 全面眼科視光檢查之費用。
  5. Bowtie 及眼鏡 88 對此推廣活動保留最終決定權,並有權終止或用同等價值及/或規格的獎品代替原先全部或部份健康服務及優惠而不作事前通知。
  6. 客戶須於指定日期內換領此服務,否則該名額將作廢無效,並不獲補發。
  7. 任何因電腦、網路等技術問題導致投保人所遞交的資料有所遲延、遺失、錯誤的情況,Bowtie 毋須負上任何責任。所有與投保有關之日期及時間(包括但不限推廣期)均以 Bowtie 的時間紀錄為準。

Bowtie Comprehensive Eye Examination Terms and Conditions

  1. The Optical 88 Comprehensive Eye Examination is provided by Optical 88 Ltd. ("Optical 88"), which the gift is sponsored by Bowtie. Customers can enjoy the Eye Examination according to the instructions in the redemption email. Terms and conditions apply:some text
    1. The Eye Examination is only available to Eligible Policy insureds aged 4 or above and is non-transferable.
    2. The Eye Examination includessome text
      1. Ocular and general health history
      2. Refraction test
      3. Intra-ocular pressure measurement (By non-contact tonometry)
      4. Binocular vision Assessments (E.g. stereopsis and accommodation test)
      5. Colour vision test (By Ishihara test)
      6. External ocular health and fundus examination
      7. Consultation and recommendation
      8. Detailed ophthalmology report (only applicable to comprehensive ophthalmology examination)
      9. Summary of optometrist’s clinical findings and observations from exam, includes retinal photo when requested by patient, approximate 1-week processing time. (report provided upon request)
    3. The Eye Examination is subject to the actual operating conditions of Optical 88. Customers need to make an appointment in OPTICAL 88 Professional Eyecare Centre according to the instruction in the redemption email sent out from Bowtie, and must present the redemption email and the QR code upon redemption. 
    4. The Eye Examination may involve pupil dilation, you may experience blurry vision after the comprehensive eye exam. You Should follow the instructions / advice and terms & conditions which may apply from their professionals.
  2. The Eye Examination cannot be exchanged, returned, exchanged for other gifts, or used in conjunction with other promotional offers.
  3. Bowtie is not the provider of the Eye Examination, which is provided by Optical 88. Bowtie is not responsible for any liability arising out of The Eye Examination or other services provided by Optical 88, nor for any problems or complaints about the service or its related service quality, please contact or inquire directly with the relevant supplier or third party.
  4. If the policyholder cancels or surrenders the Eligible Policy before the end of the first policy year, Bowtie shall have the right to recover the fee of the Eye Examination. 
  5. Bowtie and Optical 88 reserve the right of final decision on this promotion and reserves the right to suspend, terminate, substitute the Eye Examination with an alternative gift with equal monetary value and/or specification and change the details of the Offer without prior notice.
  6. Customers need to redeem the Eye Examination within the designated period. Otherwise it will be invalid and will not be reissued.
  7. Bowtie will not be liable for any matter or loss in relation to submission delay, loss, and any information transmission error due to technical problems including but not limited to any computer or internet network issues. All dates and times related to the Offer (including but not limited to the Promotion Period) are recorded according to the reception of Bowtie’s server and computer records.
