Bowtie x Optical 88 AI Eye Health Screening Terms and Conditions
The AI Eye Health Screening is provided by Optical 88 Ltd. ("Optical 88"), which the gift is sponsored by Bowtie. Customers can enjoy the AI Eye Health Screening according to the instructions in the redemption email. Terms and conditions apply: a. This service is only applicable to the eligible insured person or two immediate family members of the insured person. The two immediate family members of the insured person can also include the policyholder. b. This service is conducted by an automated system with a fully automatic fundus* camera, sensor automation system, and voice navigation system to perform fundus imaging. Through deep learning technology, the system can automatically identify structural features in the fundus images and grade the relevant health risk using artificial intelligence algorithms. Customers can obtain the report by scanning the QR code using their mobile phones with instructions indicated. *The fundus refers to the posterior part of the eye, which includes the important ocular structures such as the macula, retina, optic disc, and retinal blood vessels. Some common eye diseases, and even cardiovascular diseases, have the potential to cause abnormal changes in the fundus. c. The automated risk grading includes i. Diabetic Retinopathy ii. Glaucoma iii. Severe Cataract iv. Cardiovascular Health (Results of this item are only applicable to participants within 40-80 years old) d. Limitations of this service: i. Individuals with pupils smaller than 3mm or droopy eyelids may affect the quality of the images captured. ii. Good quality fundus images may not be obtained for individuals with strabismus, nystagmus, or IOL implantation. e. This service is subject to the actual operating conditions of Optical 88. Customers need to make an appointment in advance with a designated OPTICAL 88 store according to the instruction in the redemption email sent out from Bowtie, and must present the redemption email and the code upon redemption. f. This service should not be taken as a substitute for professional medical advice. You should consult your primary healthcare provider or medical institution regarding your specific health needs.
Bowtie will send out a redemption email to the registered email of the policy holder within 45 days from the effective (in-force) date of the Eligible Policy. The eligible customer and their designated family members or friends can enjoy the AI Eye Health Screening service during the valid period indicated and by following instructions shown in the email.
The validity period of the AI Eye Health Screening is clearly stated in the redemption email. Customers must redeem the service within the stated validity period, otherwise the quota will be forfeited and will not be re-issued.
The AI Eye Health Screening cannot be exchanged, returned, exchanged for other gifts, or used in conjunction with other promotional offers.
Each service voucher (or voucher code) can only be used by one person and be used once, and will be immediately invalidated after use.
Bowtie is not the provider of the AI Eye Health Screening, which is provided by Optical 88. Bowtie is not responsible for any liability arising out of the AI Eye Health Screening or other services provided by Optical 88, nor for any problems or complaints about the service or its related service quality, please contact or inquire directly with the relevant supplier or third party.
If the policyholder cancels or surrenders the Eligible Policy before the end of the first policy year, Bowtie shall have the right to recover the fee of the AI Eye Health Screening.
Bowtie and Optical 88 reserve the right of final decision on this promotion and reserves the right to suspend, terminate, substitute the AI Eye Health Screening with an alternative gift with equal monetary value and/or specification and change the details of the Offer without prior notice.
Bowtie will not be liable for any matter or loss in relation to submission delay, loss, and any information transmission error due to technical problems including but not limited to any computer or internet network issues. All dates and times related to the Offer (including but not limited to the Promotion Period) are recorded according to the reception of Bowtie’s server and computer records.
