What documents do you need?

After treatment, ask your medical provider to fill out and provide the original versions of the following:
1 Claim Application Form (filled in by the doctor before discharge)

We’ll review your claim based on the Claim Application Form filled out by your doctor before discharge. Download the Hospital Claim Application Form

2 Medical Receipts

Your medical receipts are used to calculate your reimbursement amount.

3 Medical Reports (if any)

Medical reports help us better understand and assess your claim. These may include:

  • Surgical Reports (e.g. colonoscopy, gastroscopy)
  • Lab Test Reports (e.g. blood tests, urine samples, pathology)
  • Scans (e.g. MRI, PET-CT)
  • Imaging Reports (e.g. angiography, ultrasound, ECG, X-rays)
4 Other Documents

If there are any additional documents related to your hospitalization or treatment, please provide them to help us better assess your case. For example:

  • Reimbursement estimate or claim breakdown from other insurance companies (if you’re also claiming elsewhere)
  • Referral letter from your attending doctor
Even if your check-up shows nothing serious, you can still claim the cost.
If your doctor recommends a diagnostic imaging test* and the results show no issues, you can still claim by submitting the required documents online.

Cashless Service Available at All Private Hospitals

No deposit at admission, no payment at discharge

If you're admitted to a private hospital, you can apply for Cashless Service to ease your financial burden. Just focus on getting better.

Log in to the Bowtie platform and download the Pre-Approval Form. Ask your attending doctor to fill it in.

Apply for Pre-Approval
Upload the completed form on the Bowtie platform. Results take at least 5 working days. We may request extra documents if needed.

Admit to Hospital
At check-in, tell the hospital you have pre-approval. Show your ID, Bowtie’s approval letter, and the hospital’s admission note.

Discharge Without Payment
Bowtie will pay the approved medical fees directly to the hospital.

Service Guidelines

Important Notes:
  • Cashless service applies to all Bowtie products (except Cancer Fighter) and all private hospitals and affiliated day surgery centers — not clinics.
  • It’s only available for non-emergency treatments, surgeries, or hospital stays.
  • Not applicable for emergency admissions.
  • Room upgrades are not covered.
  • Personal expenses like companion fees, newspapers, TV, phone, meals, etc., must be paid by you unless pre-approved.
  • If the final medical expenses exceed the pre-approved compensation amount, you will need to pay the excess amount yourself upon discharge and keep the relevant receipts. After discharge, you can apply for a reimbursement of the out-of-pocket medical expenses through the Bowtie online platform, following the standard claims procedure.
Changing treatment plans
Terms & Conditions:
  • Cashless service is an administrative arrangement, not part of your policy coverage. Bowtie may cancel it at any time without notice.
  • It does not apply to non-covered treatments or stays.
  • The actual pre-approval notice date depends on how quickly all documents are submitted.
  • Approval does not guarantee payment. Final decision depends on bills, policy terms, and exclusions. If any costs are not covered or exceed limits, you must pay them.
  • Cashless service is provided by a third-party provider. Bowtie does not guarantee or take responsibility for the quality of the service or any legal liabilities from it.

Prepare Documents

What documents are needed?

A family member or beneficiary can collect the following documents:
1  If the death occurred in Hong Kong
  • Original Death Certificate
  • Copy of the Cancelled HKID of the insured (the Immigration Department cancels it when issuing the death certificate)
  • Copy of the Beneficiary’s HKID
  • Proof of relationship between the beneficiary and the insured (e.g. birth certificate or marriage certificate)
  • Relevant medical documents issued by the hospital/doctor (if any)
  • Authorization letter signed by the beneficiary to request medical reports from the hospital/doctor (if applicable)
  • Authorization letter from the beneficiary if a third party is handling the claim (if applicable)
2 If the death occurred outside Hong Kong – In addition to the documents above, you will also need:
  • "Proof of HKID Cancellation" issued by the Hong Kong Immigration Department
  • Proof of household deregistration (if the insured was a Mainland China resident)
  • Original Death Notarization Certificate (if the death occurred in China) / Official death registration certificate from the respective country

If you've been diagnosed with another serious illness not listed , or are applying for a claim under the Children's Growth Term CI, please contact our claims specialist at 3001 5670 or claim@bowtie.com.hk to obtain the claim forms.

2 Medical Reports (if any)

These help us better assess your claim. They may include:

Life Insurance

Documents Required – Death in Hong Kong:
