Group medical insurance: what Hong Kong SMEs should know before buying for the first time

Buying medical cover for your team for the first time and not sure where to start? This page covers the basics: what group medical insurance actually covers, whether your company qualifies, and what to compare before you choose. About five minutes to read.

What is group medical insurance?

Group medical insurance (also called corporate medical insurance) is taken out by an employer for its employees as a group. Eligible medical costs from illness or injury are reimbursed according to the plan. The company pays the premium, and employees are not individually underwritten.

What does group medical insurance cover?

Actual coverage, benefit limits and exclusions depend on the series and plan level chosen. Please refer to each plan's benefit schedule and the policy provisions.

Hospitalisation and surgery

Eligible costs for hospital stays and day surgery, reimbursed on an indemnity basis. Cashless admission is not automatic: it requires a pre-approval application submitted to and approved by Bowtie before admission, applies to eligible planned inpatient treatment, and does not cover amounts above the plan's benefit limits or items the plan does not cover, which remain payable by the member§

Outpatient

General practitioners, Chinese medicine and specialists; some plans also cover chiropractic and physiotherapy

Wellness

One health checkup and one flu vaccine per policy year

Dental

Oral examination and scaling; the Pro Care Series also covers accidental denture treatment, filling, extraction and dental X-ray

Geographic scope

Worldwide. Eligible costs incurred outside Hong Kong are reimbursed according to the plan's benefit items and limits

Key exclusions

Not everything is covered. Treatment of pre-existing conditions is generally excluded, and expenses already reimbursed by another insurer or third party are not covered. The full list of exclusions is set out in the policy provisions.

What to compare if you're a first-time buyer

Premium aside, these 5 determine whether your team can actually use the cover.

01How the inpatient limit is structured

Check whether it is a single aggregate limit or a set of itemised sub-limits. Bowtie plans carry a total inpatient benefit amount (HK$25,000 to HK$120,000 by plan level) and reimburse 80% of eligible costs. The Basic Care and Pro Care Series adjust the proportion by ward class: 100% for a standard general ward, 50% for a standard semi-private room and 25% above that.

02What makes outpatient cover usable

Three numbers to ask about: the co-payment per visit, the annual visit cap, and whether cover is limited to a medical network. Network clinics cost less but narrow the choice of doctor; out-of-network gives freedom but usually carries a co-payment.

03Whether preventive benefits are included

A health checkup and flu vaccine are benefits employees use every year, unlike hospitalisation. Not every plan on the market includes them².

04Whether your company meets the threshold

Minimum team size, company age and the age limit for insured members all determine whether you can enrol at all. Worth confirming before comparing benefits in detail.

05How pre-existing conditions are handled

Under Bowtie's group medical plans, pre-existing conditions are generally not covered. The exception is where the company was insured under another insurer's group medical package product immediately before the Bowtie policy took effect. They also become covered once a member has been continuously insured for more than one year.

Five things people get wrong about group medical

What people say
What Bowtie says
"We're too small to qualify"
From 3 employees.
"Group medical is expensive"
The Basic Care Series starts at HK$98 per member per month*. A 3-person company pays around HK$3,500 a year.
"You need an agent or broker to arrange it"
Bowtie handles enrolment online, with a Bowtie group medical specialist supporting you 1-on-1 from enrolment through to claims.
"Every employee has to be underwritten first"
Group medical generally does not require individual underwriting. Only companies of 3 to 4 employees applying for the Pro Care Series need to submit a health declaration.
"Our staff have their own insurance, so we don't need this"
The two work better together. Group medical pays first and a personal or VHIS policy covers the rest, so staff pay less out of pocket on each claim. The same expense isn't paid twice, but together the cover goes further.

Does your company qualify?

Company age

Established for at least 1 year

Minimum team size

From 3 employees

Age of insured members

Employees and spouses aged 75 or below; children from 15 days to 25 years

(those aged 18–25 must be in full-time study)

Employment type

Full-time, part-time, contract and probationary employees can all be enrolled.

Which employees to include is the company's own decision.

Five reasons to arrange group medical now

01

A benefit that is affordable to add

Medical cover is one of the more affordable benefits an SME can add to its package. It gives a smaller company something concrete to offer alongside salary, without having to match larger firms on pay.

02

Cover that is simple to use

When cover is in place and claims are simple to submit, using it is one less thing for staff to weigh up before seeing a doctor.

03

Medical spending becomes predictable

Reimbursing staff medical bills directly out of company funds looks simpler at first, but the amount is hard to budget for and rises as costs rise. A group plan converts that into a fixed monthly figure per member.

04

The admin is lighter than most people expect

Enrolment, member changes and claims are all handled online. Once the policy is in effect, adding a new hire takes as little as 5 minutes^, and employees submit their own claims without HR in the middle.

05

Premiums are generally a deductible business expense

Group medical premiums an employer pays are an employee-benefit expense incurred in running the business, and can generally be treated as an operating cost and claimed as a deductible expense for profits tax. The deductible amount depends on your company's finances and the Inland Revenue Ordinance.

First-time buyer FAQ

Usually once you begin hiring full-time staff and need benefits to compete for talent. Bowtie requires a company established for at least 1 year with a minimum of 3 employees.

Yes. Employees' compensation insurance is legally required and covers work injuries and occupational disease. Group medical is voluntary and covers everyday, non-work-related medical needs. Neither replaces the other.

A package plan has fixed benefits and published premiums, so it is quick to arrange and easy to budget for. If your team has more specific needs, Bowtie also offers flexible combinations and tailor-made options, with a group medical specialist recommending a plan based on your team size, needs and budget.

Bowtie plans range from HK$98* to HK$898† per member per month, depending on the series and plan level. Premiums are based mainly on the plan level and the number of employees enrolled. Website prices are estimates; the actual premium is confirmed once your team's details are provided. For a 3-person company on the Basic Silver plan, that works out at roughly HK$3,500 a year.

Yes. Eligibility is based on age and employee status rather than employment type. Any eligible employee of the company can be enrolled, and which employees to include is the company's own decision.

Yes. An employee's spouse (aged 75 or below) and children (15 days to 25 years, with those 18–25 in full-time study) can join the same plan at a uniform per-member premium.

Once the company policy is in effect, HR adds the new member on the employer portal and coverage can be activated in as little as 5 minutes^.

HR removes the member on the employer portal and their cover ends immediately. The premium is adjusted on a pro-rata basis at the end of the policy year. A departing employee who wants to stay covered would need to take out individual medical insurance.

Members can be added or removed on the portal at any time. A new member's premium is pro-rated from their join date to the policy anniversary, and the total is settled at the end of the policy year.

Group medical premiums paid by an employer are an employee-benefit expense incurred in running the business, and can generally be treated as an operating cost and claimed as a deductible expense for profits tax. The deductible amount depends on the company's finances and the Inland Revenue Ordinance. Consult your accountant or tax advisor; the Inland Revenue Department's final assessment prevails.

No. Employees submit claims themselves on the online portal and Bowtie's claims specialists follow up directly, so HR does not act as an intermediary.
