Eligibility requirements
  • All Associate / Spouse: Age 75 or below; Child: 15 days to Age 25
  • Coverage may be extended to Insured Persons up to Age 75 if such Insured Persons had been insured under this Plan before attaining Age 65.  For Insured Persons Aged 65 to 75, a Premium Loading and/or Case-based Exclusion(s) may be imposed.
  • A child between the Age of 18 and 25 must be a full-time student with valid proof accepted by us.
*The attaining age is based on the age of employees and their dependents at the policy effective date or at the policy anniversary date.
Benefit Period
  • 1 year – yearly non-guaranteed renewal
  • Individual underwriting is required for age 70-75, insurability subject to underwriting result.

Plan Levels
All Employees under the same company shall be enrolled into the same plan level. If the eligibility requirements are not met, you can purchase a supplementary level insurance plan with no age limit for the unqualified members.

Exclusions

  • Solely diagnostic procedures ;
  • Infection with any Human Immunodeficiency Virus (HIV) and/or any HIV-related illness including AIDS and/or any mutations, derivations or variations thereof ;
  • Vaccinations, immunization, injections, preventive medication or preventive care;
  • Birth control or reversal of birth control; sterilization and sexual dysfunction, diagnostic of pregnancy or resulting childbirth, abortion, miscarriage or any complications from the above;
  • Congenital conditions that gave rise to signs or symptoms or was diagnosed, before the Insured Person attains seventeen (17) years of age;
  • Treatment for which expenses have been reimbursed under any law, or medical program, or insurance policy provided by any government, company or other third party;
  • Drugs, suicide and illegal activities.

Key Product Risks

  • Renewal or cancellation right — We reserve the right not to Renew this Plan and to revise the premium payable under this Plan on the date of any Renewal. We have the absolute right to cancel or terminate this Plan at any time by giving you notice by electronic means to the latest contact you have notified to us not less than thirty (30) days in advance. The effective date and hour of cancellation or termination stated in the notice shall be considered the end of the Plan period. Premium adjustment (if any) and the unearned portion of the premium at the time of cancellation or termination shall be paid by (or refunded to) you.
  • Product features revision — We reserve the right to revise the terms and benefits upon policy renewal by giving 30 days’ advance notice.
  • Premium adjustment risk —  Standard premium rates are not guaranteed and are subject to change based on our emerging experience in relation to claims, persistency and expenses, medical cost inflation and any change in the benefit structure. Therefore, renewal premiums may be higher or lower than the premium currently reflected.
  • Credit and solvency risk — The payment of benefits under this plan is subject to Bowtie’s credit risk and solvency. In the event of Bowtie’s insolvency, you may lose the coverage stipulated in this plan in addition to any premiums you have paid.
  • Inflation risk — Due to inflation, the costs of medical diagnoses and treatments may rise and the amount of benefit payable may become insufficient to meet your future medical needs even if our full contractual obligations are met. You are advised to consider the likely impact of future medical cost inflation when choosing a plan.Where permitted under the relevant regulations, benefit amounts and/or standard premium rates of this plan may be revised by us from time to time to reflect the impact of medical inflation
  • Termination — This policy will be terminated on the earliest of the followings :
    a. non-payment of premiums after 31 days from the premium due date;
    b. the number of Employees or full-time Employees enrolled (as applicable) (excluding the Basic Bronze Level) becomes less than three (3).Termination of this plan shall not affect any claim arising prior to such termination unless otherwise stated. Any premium paid after the termination of this plan shall not create any liability upon us but we will refund any such premium without interest.

FAQ

What outpatient services are covered?

The plan covers General Practitioners, Traditional Chinese Medicine (TCM), and Specialist visits at network clinics. The maximum number of visits in aggregate per Policy Year is 15, with caps of 10 GP, 10 TCM, and 5 Specialist visits.

What outpatient services are not covered under this plan?

Dental, chiropractic, physiotherapy, diagnostic imaging & lab tests, and psychiatric-related treatments are not covered under this plan.

How does the 100% in-network reimbursement work?

When employees visit a network clinic, Bowtie settles the consultation fee directly with the provider. The employee only pays the co-payment amount at the clinic.

What are co-payments and how are they applied?

Co-payments are fixed amounts paid by users at the point of service: HK$50 for General Practitioner and Traditional Chinese Medicine visits, HK$100 for Specialist visits under this Group Outpatient plan.

What happens if my employee visits a non-network provider or non-network doctor?

This plan provides network-only coverage. Visits to non-network providers or non-network doctor are not covered or reimbursed.

Can my employees' family members be covered?

Yes. Spouses (age 75 or below) and children (from 15 days old up to age 18, or 25 if a full-time student) can be covered under the plan. The premiums will be calculated per member.

What is included in the annual health check-up?

The annual health check-up includes 10 test categories: Blood Pressure, Complete Blood Count, Renal Function, Liver Function, Lipid Profile, Diabetes Screening, Gout Screening, Urine Analysis, Thyroid Function, and Bone Metabolism. Before undergoing the health check-up, 9-hour fast is required for Gout Screening; water is allowed. The minimum age for undergoing the health check-up is 15 years old.

What is BowtieGo and how do my employees use it?

BowtieGo is Bowtie’s app for finding network clinics, booking telemedicine consultations, and managing claims — all digitally, with no paper forms.

Can I add new employees mid-year?

Yes. Please contact our team to arrange adding new employees to an existing policy during the benefit period or use the self-serve member movement feature in employer portal.

Are pre-existing conditions covered?

Coverage for pre-existing conditions is subject to the policy terms and conditions. Please contact us for details specific to your team’s situation.

Is there a waiting period?

No, there is no waiting period for this plan. Once the policy is active, your team can access network medical services and enjoy member rates right away.

What happens at renewal?

The plan has a 1-year benefit period with yearly renewal. Our team will contact you ahead of time to help you with the renewal process.

In case of any personnel changes in the company during the policy year, how will it be reflected in the premium?

If the number of insured members changes during the policy year, a premium adjustment will be paid by (or refunded to) you to reflect the change by the end of each policy year. As employers or HRs, you may log in to our employer portal to update the member list at any time.

Is there a minimum requirement for the number of employees?

To apply for Bowtie Corp Outpatient Insurance, a minimum of 5 employees is required.
