Health Insurance for Expats and New Arrivals in Hong Kong

Those systems use different rules. Being accepted for insurance does not establish public-hospital fee eligibility, and having a company medical plan does not mean it will cover every private bill.
Can non-permanent residents apply
Bowtie's application FAQ, updated 15 October 2026, says non-permanent residents can apply if both policyholder and insured person meet its requirements.
It lists accepted identity or residence documents, including ordinary or permanent HKID and qualifying residence visas or entry permits. Both people must currently live in Hong Kong with a Hong Kong address and be in Hong Kong when submitting the application. A particular Document of Identity category has an additional restriction stated in the FAQ. [S21]
This gives a more useful answer than “ask again after your HKID arrives”. If you have a residence permit but no card yet, check that your exact document and circumstances meet the current requirements. Do not assume a visitor's permission qualifies.
The published insured-person entry range is 15 days to age 80. Health underwriting still applies. [S19]
What health information to prepare
Have your medical history from your previous country ready, including medicines, tests, consultations and follow-up advice requested by the application.
For most Bowtie VHIS applicants, the process uses health declarations rather than routine examinations. Pink applicants aged 66–80 have specified clinic assessments. An insurer may still need medical reports or clarification. [S20]
Answer using the information required, even if a condition was treated overseas. Where a question is unclear, obtain a written clarification instead of assuming a foreign record is irrelevant.
Check your company plan on arrival
Ask HR for the benefits schedule and the actual cover dates. Some employers cover staff immediately; others have different enrolment or probation arrangements.
Inspect surgical and miscellaneous limits, ward assumptions, overseas care, dependants and what happens on resignation or a career break. A generous-looking annual limit may coexist with smaller item limits.
If a spouse or child is covered as a dependant, find out whether their cover ends when your employment ends and whether they have different benefits from yours.
How a personal top-up can work
Pink's supplement allows reimbursement under other insurance for expenses payable under Pink to reduce its remaining annual deductible. It is the eligible amount already reimbursed, not simply the amount the company insurer declines to pay. [S11]
For a hypothetical HK$120,000 fully eligible bill, assume a company policy pays HK$30,000 and Pink starts with a HK$50,000 annual deductible. The company reimbursement leaves HK$20,000 of deductible. With no other adjustments, Pink could pay HK$70,000 of the HK$90,000 unpaid bill and the patient HK$20,000.
If the job ends, the personal policy can continue under its terms, but company reimbursement may no longer offset the deductible. Choose an amount you could fund yourself.
Cover when visiting home or relocating
Read territory and residence-change provisions separately. A policy may cover some overseas treatment without offering the same benefit level in every place.
Pink's enhanced cover generally excludes the United States from full reimbursement, but its supplement provides a Standard-Plan-level calculation for eligible US expenses. Non-designated Mainland China hospitals also have specific reduced-benefit arrangements. Calling all of this simply “US not covered” is misleading. [S11] [S18]
For a permanent move, notify the insurer and obtain written confirmation of continued cover, any premium adjustment, territory restrictions and claim arrangements. Do not assume an answer about a short holiday resolves a change in residence.
Public healthcare is a separate eligibility check
The HA publishes eligible-person categories and document requirements for subsidised services. Ask it how your own status and documents will be treated before relying on a subsidised fee.
Do not treat an eligible residence visa for a private-insurance application as proof of every HA entitlement. The physical HKID and any interim documentation also need to be considered under the HA's rules. [S25]
Tax relief for eligible premiums
If you are assessed to Hong Kong salaries tax or personal assessment and meet the conditions, qualifying VHIS premiums may be deducted. The cap is HK$8,000 per taxpayer for each insured person per year of assessment.
The claimant, policyholder or qualifying spouse, specified-relative definition and insured person's identity conditions all matter. The rule is not simply “only the named policyholder can claim”, nor does paying for any overseas parent automatically qualify. [S22] [S23]
If you are leaving Hong Kong, retain premium statements and check the treatment in your final assessment rather than assuming a whole year's premium is deductible.
Your first-week checklist
Confirm company cover dates, obtain its schedule, check public-care eligibility separately, gather health records and accepted application documents, and decide who needs personal cover. Then compare actual offers, including any exclusions and deductibles.
